U.S. Passport Application Required Materials

NOTE: There are absolutely, positively no exceptions to what is detailed here.  Everything must be completed EXACTLY as described, and each item is required. For additional information, reference the U.S. Department of State website.
  1. Passport Application Form DS-11 – UNSIGNED – you can complete this through the online form or by hand:
    1. For the online form,
      1. Follow the instructions and complete accordingly. Note: You are applying for a passport book.
      2. Print hard copy to bring with you
      3. DO NOT SIGN.  It must be signed in the presence of a Passport Agent (i.e. at Passport Day).
    2. For completing the hard copy form by hand:
      1. Print the application form and complete ahead of time in black ink only,
      2. DO NOT SIGN.  It must be signed in the presence of a Passport Agent (i.e. at Passport Day).
  2. Proof of US Citizenship (provide only ONE of the following)
  • Certified US birth certificate – photocopies CANNOT be accepted
  • Consular Report of Birth Abroad or Certification of Birth
  • Certificate of Naturalization or Citizenship
  • Previous fully valid US passport book or card; can be expired
  1. Proof of Identity (provide only ONE of the following)
  • Current, valid driver’s license
  • Government employee ID
  • Military ID
  • Recently issued Certificate of Naturalization or Citizenship with easily recognizable photo
  1. Passport Photo
  • Must be an official passport photo.  Photos made at home or cut from previous pictures CANNOT be accepted.
  • Official passport photos can be obtained at Terrapin Shipping and Mailing in STAMP, CVS and Kinkos in College Park, or at most passport-issuing locations, such as your local Post Office.
  1. Photocopy of Valid Photo ID - Clear photocopy (black and white is OK) of the front and back of the valid photo ID that is being presented

  2. Payment (only for students who are awarded the Processing Fee Grant) for $110 Application Fee
  • Checks (personal, certified, cashiers, travelers), payable to "U.S. Department of State"
  • Money orders (U.S. Postal, international, currency exchanges) and bank drafts, payable to "U.S. Department of State"
  • The $25 processing/execution fee is paid by UMD
